Résumé : La première prescription de verres sphériques convexes de + 1 dioptrie
permet au jeune presbyte de revoir net à 33 cm. Le centre supérieur de
l'accommodation-convergence n'a plus à ordonner que 2 dioptries d'accommodation et donc 2
angles métriques de convergence. Le sujet voit net à 33 cm et converge à 50 cm.
Nous avons provoqué une dissociation accommodation-convergence. La convergence proximale
et la convergence fusionnelle doivent alors compenser cette insuffisance de convergence
iatrogène. Toute hétérophorie, toute neutralisation pré-éxistante sont alors
aggravées.
Seule l'orthoptique peut rendre le confort perdu et la possibilité d'une adaptation aux
verres simples puis multifocaux.

Summary : With the prescription of + 1 sphere the young presbyope will see clearly
again at 33 cm. But, the cerebral accommodation-convergence center will order only 2
diopters of accommodation, thus giving 2 meters angles of convergence. The subject will
see clearly at 33 cm but converge at 50 cm.
We have effectively created a dissociation of accommodation and convergence. This may be
compensated by the mechanism of proximal and fusional convergence, but, any pre-existing
heterophoria or neutralisation may be aggravated.
There is an important role for orthoptics to help the young presbyope to adapt confortably
to the use of a new near correction.
